How to get gig info from buyer

Hi Guys

 

AM new to fiverr and just setting up my first gig. I have done a form in a google doc that i want to give when someone purchases my gig. This asks them for all the information i need to complete the gig and they can then re upload it to fiverr. Where can i upload the template so the buyer can get it?
