How i improve delivery on time rating from 50 percent to 90 percent?

kindly someone guides me on how I improve my on-time delivery from 50% to 90%. due to some reasons, my on-time delivery goes to 50%. basically I request the client to extend the delivery time after that in some minutes I delivered the project then client accepts the request after the delivery. I realize i already delivered the project but I get the notification you are late to deliver on time. I again deliver the project then it shows the order is completed. that’s why my on-time delivery goes down. now what I do to increase my on-time delivery.
