If your account has been suspended or is under review, you should have received an e-mail from Fiverr about it. 

You can create a ticket with support here: https://www.fiverr.com/support_tickets/

If you can't sign in to your seller profile, you can send an e-mail to support@fiverr.com

But like I said: you should already have an e-mail explaining why your account is under review or suspended.
