How to withdraw Fiverr Pro Application?

Hi everyone,

I am suffering from a problem to apply for Fiverr pro application. Unfortunately, somehow I did complete my application and sent it as it is. Now I can see no option to withdraw or to apply from a whole new form. Kindly guide me regarding the solution of this problem.

I did an error submitting the application of Fiverr Pro; now I could not find the option to withdraw the application. 

I have been waiting to reject the application for the last 3 months but, to no avail. 

Kindly suggest me the solution.
