Wow, that's awesome advice. you are right achieving Rank isn't nothing do with fiverr business. Rather than reacting on post we should focus on business. I used forum after two years of joining fiverr, purpose was to coming here to get a solution regarding the fiverr violation of term and conditions. We can't upload more than three Image, but we have also addition two for pdf where you can upload your proof of work, PDF will attract your buyer, they will know what kind of services are providing them. Other seller might have image more than 20, but those aren't upload by the seller. When you deliver your order you will get option to upload proof of work, there will be option to share those proof with people, by default it might not be enable. Please be careful while selecting those thing because if you are delivering sensitive or personal thing that will not be good for buyer.

I can understand your concern. First think, how much would you pay to someone as a buyer, if you think 20$ is fair enough or just 5$ is enough? It's up to you how much you want to charge for a cover design, how much time you have invested in learning of cover design? As far concern is about book cover design, you should also mention about what kind of cover you are going to provide with your gig, like paperback, hard cover or something else.

I provide a service to my client where client has to pay to other services (website) can i say my client to pay them directly or should i ask them to pay me on fiverr? If i will tell them to pay to direct services provider and pay me only for my own charges. Will it violate fiverr term and condition?
