How do you write your resume as a freelance voiceover artist?

I need to write a resume using some of the companies I’ve worked for through this site but I’m not sure how that would work. The format is column 1- company, 2-role and 3-production company. Since the production company is taken out because of Fiverr, what do you fellow freelancers put as your resume information?

Before anyone tries to say it’s not needed for freelancing or anything, it’s for an acting resume, and looks a bit blank without the production company (or director).
