Chatting on other platform rather than fiverr??

If someone ask you to chat on other platform rather than fiverr, is it consider a scam.
And also this person was asking for logo creation and send me form to fill out, and in that form was included the social security number or a government id.
Also this conversation was going on telegram not on the platform it self.
I need your help on this one. Is he trying to scam me or he is a regular costumer.
Pls Help!

SCAM 100%. Don't share that kind of personal info over the internet.

Again scam. Anything that encourages you to go away from the platform, that's stuff to avoid.
