I could not withdraw with paypal

Hello i have join fiverr since 2011

my user name: dollarsman1982

i have no active for a long time and last month ago i have new order with balance $4 in my account and cleared.

I have follow instruction here to get paid by paypal http://support.fiverr.com/hc/en-us/articles/204060786-Withdrawing-Funds

but i did not receive any email confirmation from fiverr to the next step.



any body help me to solve this issue
