Bug at Forum and Customer Support doesn't work anymore

Ok, I wanted to submit this at customer support, but if I want to submit something it says: Your username: Cannot be blank, Order number: Cannot be blank. Ehm, I can’t input my username or order number anywhere.



As for what I wanted to report, I will copy what I have typed:

Hello Fiverr,



I couldn’t choose the correct Help Topics for Technical Issues category, so I choose one of the first ones.



I was using the Forum today and if I press at Log Out or my username, I need to click just beneath the word. I think there are some problems with the CSS here, as it doesn’t work completely correct for my browser (Google Chrome). Just wanted to report it to you, I don’t really have a big problem with it, but this bug could be a bit annoying for other users.

There is an issue with “Technical Support” only. You should be able to submit tickets using other classifications if necessary using this link: http://support.fiverr.com/hc/en-us/requests/new

I am having the same issue, getting "Your Username: cannot be blank"



Well my problem is:



For the category : https://www.fiverr.com/categories/programming-tech/mobile-app-services/



My gig : https://www.fiverr.com/vivdub/create-an-android-app-for-you

is either not showing or might be after few pages, in-spite of it doing good, earlier it was showing on top.



Any help ?

Experiencing the same problem.

EVERY category of support that is requested, the same error is being reported.

I guess there is some small piece in the code which makes it not work anymore. I always hate that when I 'm coding, having a small bit which messes up your whole code.
