How to leave format when seller does not finish job?

I ordered a wordpress header from “Ravsingh”. I first contacted him and sent him a copy of my business card, saying

I wanted a replica of it for my website. He said he could do it, so I ordered the gig. He subsequently sent me two designs that are not at all what I asked for and he has refused to communicate with me after my last request for a modification. Is there any way of leaving feedback for him when the status is not complete but it has become obvious he has no intention of completing this?
