I want to make sure my site is set up properly for SEO. If I order an seo report, who will actually set it up. I need more than just a report of what is missing.

If you order an SEO report from a gig seller here on Fiverr, that seller will presumably tell you what needs to be improved. After that, it would likely be up to you to make the changes to your site, and put those suggested improvements into practice.
