I accidentally did a charge back from PayPal about a week ago that has had my friend's Fiverr account frozen.  I totally forgot that it was linked to her account because I set it up for her as a gift years and years ago to help her start her business.   She called me frantically that Fiverr shut down her account because of it so once I realized the mistake I reversed the charge back from Paypal.  Paypal said that it could take weeks for that to happen so we went ahead and signed up again for the services that we require but Fiverr locked our account.  She needs to have access to this account in order to proceed with running the business.
This whole mess is my fault and would like to do whatever I can to make it right.

There's nothing that can be done. Fiverr automatically shuts down accounts when you do a PayPal chargeback. So the account will not be reinstated.

As I said above, a refund made through Fiverr would be fine, but once you do a PayPal chargeback.. that automatically shuts down the account and there's no chance of getting it back. Sure, you can try to contact customer support, but I don't think they will be of much help here, to be honest.

Correct! Still he should contact support for better understanding. But I saw some post related to these. They also get banned.

Fiverr offers refunds via customer support or the refund system from the gig page. If you choose to do a chargeback via your bank or any other system, you automatically get banned. Why would you use someone else's account and your own PayPal there? It seems to me we only know some of the story here. Regardless, it won't matter because Fiverr won't reinstate accounts that made a chargeback. Since it breaks their TOS.

As I mentioned, i contacted Paypal to reverse the charge back. I have emailed Fiverr about the situation and I will provide any documentation that it has been reversed if they need it.
She had made many purchases on her credit card over the last year and this time for whatever reason the only option shows Paypal so she selected that one.  That was what hit my account and I saw my statement freaked out and instinctually disputed the charge.  She contacted me asking if that was my Paypal and realized what had happened.
