How do you manage your graphic identity creation bundle orders ?

Hi everyone, 

I was wondering how do you guys are managing your "package" gigs ?
To clarify, let say your gig is about creating a bunch of things as a branding, logo, font, color palet, few pictograms, patterns, business card etc ...
Before anything else you get a logo design approved by the buyer right ?  deliver the whole thing is nonsens ...

This said, do you define a limited amount of logo designs in your gig ? what if none of the designs is approved ? the whole order is "suspended" and you ask for an extra to get more ?
