I had some questions about Fiverr gig keywords.

My service will have the main keyword. Should those keywords be everywhere in the gig title, description, and hidden keywords?
Or should I use more than one? Such as gig title separate keywords, description separate keywords and hidden keywords should be given separately?

Through which gig rank will come more?

My SPM said there is no need to "double dip" keywords. Once the algorithm finds the keywords, it does not look further.

Using keywords that apply to your services is best. And they do not all need to be in your title.
