That's fiverr pro. Fiverr pro and Fiverr business were separate, are they combined into Fiverr pro now?

Yes, Fiverr Business is now Fiverr Pro (I like the regular UI better though over Pro UI, search filters are a mess in Fiverr Pro)

As of now, Fiverr isn't accepting any application from Seller for Fiverr Pro. It's currently invitation-only program as mentioned.
