If a buyer places an order without my confirmation and asks for items that are out of scope and also creates a cancellation, if I accept the cancellation it will affect my account rating.

First try to explain what you provide in the given package which he/she buy. If client is willing to pay than go ahead. Order cancelled by mutually does effect your profile you can contact customer support they will adjust your statistics.

If you mention that buyer should contact you before ordering, then you have the right to cancel it. You can ask CS to cancel it by providing a proof that buyer ordered without contacting you, and buyer's order is out of your scope. If you haven't delivered anything yet, then ask CS to cancel the order in the resolution center and it does not affect your rating.

Any buyers can place a order without asking. If its not your scope you can explain it and cancel the order. But it effect your account. If you can contact CS and explain exactly your problem after that they can get an action.
