Even if you "rank" on the first page there's no guarantee of any orders. It also depends on your skill, how you present yourself to buyers, etc. I don't understand why everyone wants to rank on the first page. That's not how Fiverr works, Fiverr offers personalized results to all clients, and they have a plethora of filters to use. So yeah, few people use regular category search anyway, which means being up there will not help you that much if at all.
