Fiver pro Application process

Hi, recently I was filling up the Fiverr pro Application form to something around 24% when this message showed up immediately:

though I don't know smapply-new.

Also, When I click the "Don't Allow" button, just a white page appears with the text "access_denied"
What does it mean? Does the "sm" phrase refers to "Survey Monkey" itself?Is it completely safe to accept the request?
I would appreciate for any help or suggestions
