Using the same bank that was used in permanently blocked account

My account was blocked due to some mushaps that were against Fiverr terms. Can I withdraw the funds to the same card that was associated with my permanently blocked account?

You should not open a new account. They will ban you once they find out either way.

Fiverr does not allow banned sellers to make new accounts. You will need to find another platform to sell your services.
