Yes, there are a few things that will result in this.

incomplete delivery, PayPal chargeback, faulty delivery, delivery is not as promised as what you listed in your gig.

buyer contact fiverr support to cancel the order for whatsoever reason

What if the file is complete , the project is good. Client thinks he got the file. And by asking refund he get to keep the project and money to himself

Some people say that Fiverr thinks of buyers as kings and this is possible.

My personal experience: When I asked for a refund, I was asked to provide proof that the work wasn't up to standard and write a short report about it. So I don't think it's that easy to get a refund.
