we often face the situation "when we upload a gig, sometimes fiverr asked to take skill assessment test. 

• My question is in which case of gig category of fiverr needs skill assessment test?

@vickieito if i couldn't make a minimum score on a test, will it hamper my gig or seller profile?

Only if it's required to publish the gig. I think you can also retake tests if you get a low score or don't pass the first time.
