Here are the steps for creating it: https://www.fiverr.com/support/articles/360010558598 You should get an email if it is or isn't approved. If fiverr says they haven't approved it, you need to get in touch with them directly. On the forum we're just buyers and sellers and don't know much more that you about the state of your account.
