fiverr affiliate cannot sign up for fiverr affiliate

I cannot sign up for Fiverr affiliate. tried different browsers, users, and incognito. But nothing seems to be working. "Next" button is always disabled and no matter what I input it does not take. What's the problem?

P.S: My fiverr affiliate credentials do not match with the seller account.
