Portfolio images not being added to all deliveries?

I notice a problem about gig port folio image. I attach a JPG image to the portfolio at the time of each order delivery. But sometimes it is added to the portfolio and most of the time it is not. What can be done in this case?

Client rejects the portfolio image, client can do it while accepting the delivery.
