In my opinion, your Fiverr handle doesn't really matter, You can brand yourself anyway you want.  Don't stress about it and just go be a great seller.

Screen name doesn’t matter, in my opinion.  But when corresponding with clients, I’d recommend using a first name to give it a more personal touch.
