US-based seller - how do I report Fiverr-earned income when filing my taxes?

Hello! I am based in the US, and have about $10,000 of Fiverr-earned income to report for my 2021 taxes. (Just guesstimating, here - it's likely a bit less).

Is there any paperwork I need to request from Fiverr? How do I get an exact number to report when filing my taxes? Thanks so much.
