It might be because he have received warning or multiple warning from fiverr. When you started receive warning from fiverr, it may drop your level.

One of my friend had the same issue, that’s why i know

Shiekhsabayyal has it spot on. There are many reasons levels can be lost and even sometimes they can be LOST for purely mistake reasons of the system.
