Sounds like a foot fetish thing. I’d consider that ‘adult services’ and refuse to do it. It’s against Fiverr’s TOS, I do believe.

Either a service that’s offered on specialized (and more expensive) sites only, and something for his own, err, erotic pleasure, or, possibly, a video clip of female feet he’d sell to other foot fetishists.
