I am designing WordPress websites in Fiverr and I have an idea to provide my clients a hosting service. simply the have to customer offer pay per month for hosting is that possible in fiverr.

No. Or, at least ‘no, unless the ToS has changed’.

Until recently, Fiverr did not permit services that took longer than 30 days. There have been changes recently that might affect or change this, but ‘monthly-basis’ stuff is still new and unknown and only permitted for certain services. I doubt hosting is one of them, but if you want to ask Customer Support for clarification, then sharing the answer they give you here would be appreciated.
