I would assume you put your questions in the RED box…

Maybe when you click the drop down menu - where it says Attachment after you have filled out the questionnaire you need to highlight by clicking on the word Attachment and then wherever it is on the page OK or continue… I do not have this on my page, so not sure what it looks like.

I don’t think there’s any way to add a PDF attachment.

You can receive a PDF attachment from the requirements that a buyer leaves, but you can’t add one.

I think the best thing to do would be typing up your questionnaire in the requirements box.
