Unfortunately, we can’t resolve this issue for you. We’re just buyers and sellers here. We hold no influence over Fiverr.

We are not fiverr support we are just sellers here on the forum and can not resolve your issue.

However fiverr banning people for breaking the rules (and usually breaking them at least 3 times) and they don’t restore blocked accounts (only if it was obvious mistake, which in your case it seems it didn’t.)
