Since there have been several people asking about this lately, here is a screenshot from today, Nov 29, 2018.

It used to be done via a checkbox below the sample image thumbnail, but it looks a bit different now and might be easier to miss.

Per default, you allow the sample to be displayed in the seller’s portfolio (checkmark in green circle shows on the sample).
If you do not want your delivery sample shown there, you need to click on the image/video thumbnail.
