Missing or incomplete order

I ordered a premium service that was to include vector, source, letterhead, etc along with five revisions. The first four revisions were fine but I did not receive the last paid for revision. Now the order is marked complete and I haven’t received anything that I paid for other than mockups. How do I get this resolved - I assume there must have been a glitch in the system.

Yeah, I know this. And, I had an outstanding request for my 5th paid revision that went unanswered and now I have nothing! I did submit a ticket so maybe the gods will smile.
